    Two passport size photographs (1) pasted on form, (2) pasted on college I.D. Card.
    Photocopy copy of Marks Sheet & Provisional Certificate
    Completely Filled Admission form & College I.D. Card.
    Bring Original Marks Sheet (class X or O LEVEL) & Original Provisional Certificate for submission of Admission forms.
