(1) APPLICATION FORM/PROSPECTUS Can be obtained from Admission Office PUMHSW From: 24-12-2018 (Monday), on Payment Rs. 2000/-.
(Two Thousands, Only) (Non-Refundable) through HBL Bank Challan PMC Branch, Nawabshah.

Candidate Can Download Hard Copy FORM from website (www.pumhs.edu.pk) OR Link (https://www.pumhs.edu.pk/files/APPLICATION-FORM-2019.pdf) and same filled FORM along with all required documents attested with Demand Draft of Rs. 2500/- (In the Favor of Director Admission PUMHSW) can be submitted through TCS at Admission Office PUMHSW-SBA.

(2) And Candidates Must Fill Application FORM ONLINE on official website www.pumhs.edu.pk & Online_Link www.pumhs.edu.pk/admission,
Online Submission is mandatory otherwise application FORM shall not be considered, and submit ONLINE Data Print Copy with FORM Hard Copy, at the time of submission.
Please Before Online Submission First Obtain PROSPECTUS/FORM OR Download Application FORM and Write Correct Application Form Number & Other Data Otherwise FORM will be rejected.
Application Form Number is Mentioned on First Page of Form.
(3) Last Date For Submission of FORM is 05-01-2019.
